Remark 1:That is the first I have heard of that. I have never been told that reporting is my sole responsibility. Remark 2:How will we do that?What do you think? Remark 3:Okay so first we need to identify exactly what is the problem and the best method of doing this is the 5 Whys?

The payroll manager complained:

1. Why?The payroll report was incorrect and missed the deadline.

2. Why?The nightshifts hours were not included.

3. Why?The report was not checked for nightshift hours.

4. Why?No employee was assigned responsibility for this task.

5. Why?There is no procedure in place.

So, are the agreed that the solution to this problem is to flowchart this payroll Report procedure?We will also display it in the office to eliminate future mistakes.
