I’m still a beginner in Chinese, but I found the following reference that explains this.

Here’s the pattern.
This is how I personally understand this structure.

It seems that a more literal translation would be something like “How long of time (between) each/one time do you [Verb]?”. So if you do X once a week, the answer to “how long of time (between) doing X one time?” is one week. Of course, it’s much more natural in English to just ask “how often?”. It appears that even though in Chinese it is literally asking for the duration between one time, it is still natural to respond by saying something like “twice a week”, etc, no need to respond “one time every 3.5 days”. This seems to be just the pattern in Chinese, and it is understood to mean “how often?”
