What does this have to do with being angry? Is this past tense, present. I am so confused on this line.

It means ‘perhaps his brother has something up with her for whatever reason’ ce l’ha con lei - is like has a gripe with or holds a grudge with her

Verb
avercela pronomial (Combination of: avere + ce + la)

to be offended or hurt
to be annoyed

ce l’ ho
ce l’ hai
ce l’ ha
ce l’ abbiamo
ce l’ avete
ce l’ hanno

Forse suo fratello ce l’ha con lei per qualche ragione.
Maybe your brother is angry with you for some reason. {polite register}

ce l’ha con = to have it with (to be angry/annoyed with) [v=avercela]
ce l’hai con me? = are you angry with me? [v=avercela]
ce l’ ho con te! = I’ve had it with you!
