I saw an ad for a Spanish language program and it featured the phrase, “Qué estás haciendo usted.” Seems like they are mixing the formal and the informal here. Is this correct grammar? It doesn’t look right to me, but there may be a special case I am unaware of.

That is incorrect. Funny that is a Spanish language program.
